If you live within a four mile radius of The Hill Medical Group (excluding the greater Belfast area),  you can register with the practice. Simply call in to the surgery between 9.00am and 12.00 noon, bringing with you your medical card and some form of identification and proof of address in the form of a recent utility bill/bank statement. If you do not have a medical card you will be asked to complete a registration form.

We will then book you a new patient medical appointment with your chosen doctor which may be a few weeks away, so we ask that if you are on any routine medication please ensure you have enough from your current doctor. The appointment  consists of a short medical, blood pressure, height and weight check, a urine test and discussion of your past medical history.

If you are currently registered with a GP in Northern Ireland and don't have a medical card print off and complete a HS200 registration form (see link below) and we will arrange a new patient medical appointment for you.

If you are currently registered in England, Scotland or Wales you need to print off and complete a HS200 (see link below) . Also required by Business Services Organisation (BSO) is a copy of photographic ID & proof of address in the form of a current utility bill/bank statement.


Paper copies are available from reception.

Please also get your Health & Care number from your previous GP.

For those born outside of the UK or not registered with a GP in Northern Ireland please call in with the surgery to collect a HSCR1 registration form
