If you live within 3.5 mile of The Hill Medical Group (excluding the greater Belfast area), you can register with the practice. Simply call in to the surgery between 9.00am and 12.00 noon, bringing with you your medical card and some form of identification and proof of address in the form of a recent utility bill/bank statement. If you do not have a medical card you will be asked to complete a registration form.
If you are currently registered with a GP in Northern Ireland and don't have a medical card you can print off and complete a HS200 registration form (see link below) and bring it in with ID and proof of address between 9.00am -12noon
If you are currently registered in England, Scotland or Wales you need to print off and complete a HS200 (see link below) . Also required by Business Services Organisation (BSO) is a copy of photographic ID & proof of address in the form of a current utility bill/bank statement.
